My surgery was done in May of 2023 by Jeff Kozak. Worth noting, I was in excellent health before going to him except for some hip and back pain. "No problem, we can fix that", he told me. The hospital stay was supposed to be 2-3 days......and one surgery. In the first surgery, Dr Kozak nicked the dura of my spinal column. After a week of me having horrible pain and headaches, he performed a second surgery to repair the damage that he had done to my dura (blood clot and spinal fluid leakage at base of my spine.) A few days after the second surgery, he came to my room and convinced me and my husband that I would 'NEVER' have to have another surgery IF I just let him do one more surgery from the anterior. I was having no pain other than a bit of surgical pain at that time, which was normal. I later discovered in the hospital notes, that he had DOCUMENTED that he performed that 3rd surgery b/c I was having pain. This was a blatant lie.... and HE knew it. He told me that I needed the surgery so that I would not need to have surgery in the future.. never a discussion of pain by me or him He, along with Dr Bahtia (Kozak's Vascular surgeon) performed the 3rd and disastrous surgery. I was left with internal bleeding, clots that formed all over my body, including my femoral arteries (R&L), clots in my R iliac artery, clots in my vena cava, in my lung, a clot in my L abdomen the size of a cantaloupe, etc, etc. I demanded to be transferred to the vascular hospital nearby b/c, at this time I had no pedal pulse in my R foot and my R leg was swollen to 3 times its normal size and I had very little feeling in that leg and foot. I needed a vascular surgeon. Dr Kozak HIGHLY resisted my transfer but with my demands, I was transferred. I was told by my vascular surgeon in the new hospital, "You didn't have this before your surgery....you have it now. This happened IN surgery". He said it was caused by an over manipulation of my internal organs during surgery. I had to have 3 more procedures in the Vascular hospital including iliac stents and pulmonary filter which saved my R leg and my life. After 28 days in the hospital and 5-6 units of blood, I went home in a wheelchair. 20 months later, I still suffer from serious neuropathy/pain in my R leg, as well as multiple other physical problems as a result of what Kozak did. I have spent every single day since going home from the hospital doing PT to try to achieve some sort of normalcy in my physical self. I suffer every single day. This man RUINED my life and I certainly will die many years sooner because of what he did to me. The original pain in my hip and back are still there BUT to a much greater degree. I would NEVER, EVER have this man do surgery on anyone who I love, EVER. If I could give negative stars to this man's review, I surely would. Sound like a horror story? It was one and it continues every day.

This facility is one of the best facilities you could ever go to. This Doctor is amazing. The people at this hospital is amazing. You will never, ever have a better experience at this place. I am not exaggerating. They are people who will make you feel like you healing and not make you feel sad. These people are wonderful. This place is wonderful. Dr Kozak is the most amazing man/doctor! He is so humble but such a talented Surgeon. He is the Surgeon who fixes others mistakes. Also, he is personable and cute. (I can say that cause I’m old). lol
But seriously, I was so blessed to have him as my doctor. I am so thankful.

Dr Kozak takes his time to review all test while explaining and asking questions as he goes forward. I love the idea that he is very personal and lets you know his goal is to make sure the patient gets the best care he can give. He sets your mind at ease. He makes sure you have a chance to ask any questions you have. He lets you patiently voice your concerns also. Trust him with my life!!

I have been a patient of Dr. Kozak’s for several years. He and his staff are very personable and knowledgeable. It’s nice when a surgeon doesn’t push surgery as a first option, but uses common sense and recommends alternatives.
I cannot say enough about the entire Texas Orthopedic Hospital. It is staffed by friendly and caring people. The hospital is clean, easy to navigate and the food is actually delicious; not your typical bland hospital food.

I had to wait a bit long to be seen but Dr. Kozak was very apologetic and gave me the reason for the wait. It was much appreciated because I have.experienced ones that just act as if you should be glad they were able to see you. So Thanks Dr. Kozak for proving them wrong!
